Innovation, Compassion, and Progress for Our Unhoused Neighbors
A Message From Our Leadership
The journey continues, and our impact deepens.
This year, innovation met proven results, data drove compassion, and every milestone brought us closer to a future where everyone has a home. In FY25, LifeMoves served 6,317 individuals and families across 25+ physical sites and additional community-based programs in Santa Clara and San Mateo Counties. But these numbers only begin to tell the story. 90% of our clients received the intensive case management and tailored support they needed to find housing, financial stability, health and wellbeing, and personalized pathways to independence.
This year brought significant expansion of our housing capacity and services. We welcomed clients to two transformative new communities–the 1111 Veterans Program and the Branham Lane interim housing community. Additionally, our San Mateo County Navigation Center expanded its integrated healthcare model to include dental services. Our outreach teams connected more than one person every single day to shelter or housing: 587 individuals and families moved from the streets to stability as a result of our teams’ efforts.
As a result of our growth, innovation, and focus on client success, 68% of families and couples we served moved directly into permanent housing, up from 57% last year. Even more encouraging: 96% of the families who exited to permanent housing were still permanently housed one year later. Structural challenges – including an aging population, sky-high rents and limited affordable housing availability – created steeper barriers for our single adults, 26% of whom secured permanent housing this year. We are encouraged that when single adults do secure housing, they succeed and we saw 90% retain their home for at least one year after leaving a LifeMoves community.
The housing crisis is only growing. For every 100 families seeking affordable housing in our region, only 24 units exist. And so our resolve and commitment to providing our neighbors with the critical interim housing and services they need is only growing too. With the generosity of our community, the competence of our partners, and the ingenuity of our services ecosystem, we will continue to deliver the support our neighbors need to thrive. We believe in the potential of every person we serve and in the power of Silicon Valley to ensure every member of our community has a safe place to call home.

The Crisis Continues
The Reality We Face
The Bay Area homelessness crisis remains one of the most pressing challenges of our time. That’s precisely why LifeMoves exists: to move our most vulnerable neighbors off the streets quickly, effectively and with dignity and provide a pathway to stability for our most vulnerable neighbors.
A PERFECT STORM REQUIRES A POWERFUL RESPONSE
The expiration of COVID-era eviction protections has pushed families into homelessness at unprecedented rates, but LifeMoves is rising to meet this moment with expanded capacity and innovative solutions.
We’re also addressing the disproportionate impact on communities of color, particularly Black, Latino, and Indigenous populations who experience homelessness at rates far exceeding their representation. Our specialized programs support aging adults and people with disabilities, meeting their unique medical and housing needs with comprehensive care.

Every challenge we witness becomes a call to action. Every barrier we identify becomes a target for innovation. And every person we serve becomes a testament to what’s possible when tailored support meets individual resilience.

Supporting Children and Families
1,169 children (ages 0-17) found stability in LifeMoves communities this year, a 2% increase from FY24. Families with children are experiencing homelessness at accelerating rates, driven by the end of COVID safety net programs and acute affordable housing scarcity.
72% of children who exited from a LifeMoves interim supportive housing community went to a permanent home. Nearly three out of four kids who came through our doors now sleep in stable homes, an outcome that breaks generational cycles and creates futures filled with possibility.
Housing Outcomes
Through innovative partnerships and expanded programming, we’re creating more pathways to permanent housing for every person who walks through our doors.
68% of families and couples exiting LifeMoves interim supportive housing—685 households—secured permanent housing, up from 57% in FY24.
26% of single adults exiting LifeMoves interim supportive housing—284 individuals—secured permanent housing, a result closely aligned with San Mateo County’s 27% benchmark for non-congregate sites.
46% of clients exiting LifeMoves interim supportive housing—969 clients —secured permanent housing, outperforming Santa Clara County’s overall benchmark of 43% for all household types across all housing models.
Strategic Expansion:
New Communities and Enhanced Services
1111 Veterans Program
In Fall of 2024, we launched a purpose-built community for veterans experiencing homelessness. With 39 fully furnished units featuring private kitchenettes and located near the Menlo Park VA Medical Campus, 1111 represents a new standard in veteran-specific interim housing. Early results are promising: we’re seeing 6-month average stays with high rates of successful transition to VA permanent supportive housing.
Branham Lane Interim Housing Community
Our 204-unit, multi-story modular community in San José began welcoming individuals and couples in FY25. Funded by a $51M state Homekey award, Branham addresses San José’s acute need with private units, en suite bathrooms, kitchenettes, community spaces, and a dog park.
San Mateo County Navigation Center Expansion
Through a partnership with San Mateo County and the University of the Pacific’s Arthur A. Dugoni School of Dentistry, the San Mateo County Navigation Center now offers an on-site dental clinic, reducing barriers to access and improving healthcare uptake among underserved populations.
Our Evolving Model
After reviewing thousands of client journeys and soliciting feedback from city and county partners, case managers and clients, we refined our service delivery model based on what the data revealed about client success. Our updated approach reflects years of learning distilled into a clearer, more empowering pathway.

Every data point represents a life changed. Here are just a few of the 6,317 stories written in FY25.
Matthew: Journey from
Encampment To Stability
After three years of living in his van at the city of San Jose’s largest homeless community, Columbus Park encampment, Matthew connected with LifeMoves. Matthew had a college education and 40+ years in skilled trades, but self-employment costs left him homeless for over a decade. A stroke and chronic conditions at age 61 ended his physically demanding work.
LifeMoves case managers provided crucial support, connecting Matthew with permanent housing and coordinating essential resources. With furniture from the Bay Area Furniture Bank and ongoing budgeting support from our team, Matthew now has stability, showing how dedicated advocacy can transform lives.
Katalina: Finding Family and a Stable Home
After arriving at LifeMoves in April 2022, Katalina received tailored medical, dental, and behavioral health support that helped her stabilize. At the San Mateo County Navigation Center, she found unexpected community: “The people here… I have made so many friends. We’re all family.”
Today, with Social Security secured and her health stable, Katalina is preparing to move into her own apartment, where rent will be just 30% of her income—her stable home.
Ines: A Mother’s Courage Creates a New Beginning
When Ines and her two daughters arrived at First Step for Families in September 2024, they had fled domestic violence in El Salvador and were ready to rebuild. Despite learning a new language and healing from trauma, Ines secured her work permit, enrolled in ESL and GED classes, earned her driver’s license, and built her savings. With support from LifeMoves, including specialized services for her daughters, the family moved into their own apartment in San Mateo.
“The number of times I’ve fallen is the number of times I’ve gotten up,” Ines shared.

LifeMoves Palo Alto
This spring, we will welcome clients to our newest interim housing community in Palo Alto, providing 88 units (64 for singles and 24 for families). The site was made possible in part through a $26.5 million Homekey grant and a strong partnership with the City of Palo Alto.
This community will feature:
- Family-friendly units with en suite bathrooms
- Community kitchen and dining spaces
- Children’s activity areas and outdoor play spaces
- Integrated behavioral health and medical services
- Pet-friendly policies
