San Mateo, CA — LifeMoves, Silicon Valley’s largest provider of interim and supportive housing, and Hearts for Homeless, a Bay Area-based volunteer organization, have jointly launched a Pilot Transition Support Program designed to provide continued assistance to San Mateo County Navigation Center clients transitioning into permanent housing.
The pilot initiative aims to bridge the critical gap that too often follows a positive exit, when many individuals with lived experience struggle with isolation and risk returning to homelessness. This collaboration represents a new community-driven approach to extending emotional, social and practical support to ensure long-term housing stability and social re-integration.
“Ending homelessness involves more than housing solutions — it requires building a foundation where anything else can become safely relevant,” said LifeMoves CEO Aubrey Merriman. “This partnership reinforces our belief in the dignity of every human being and the resilience of the human spirit. Our care teams meet clients with whole-person support through focused services, personalized case management, housing navigation and behavioral and physical healthcare. When a community comes together to embrace neighbors and offer opportunity, compassion and presence, we build something far greater than four walls – we create a bedrock for hope, belonging and enduring success.”
Under the program, permanent housing-bound Navigation Center clients will have the option to pair with trained Mental and Emotional Support Advocates (MESAs) from Hearts for Homeless.
MESAs will provide weekly one-on-one support, help connect clients with community and faith-based groups and encourage participation in programs such as the “Connect With Your Soul” (CWYS) well-being and skills workshops. LifeMoves will provide access to client transition plans, facilitate training for volunteers and support the logistical coordination of services.
“This partnership allows us to deepen our work and walk alongside individuals as they reclaim their lives in the community,” said Jack Fuller, CEO of Hearts for Homeless. “Stable housing is the start, not the end, of recovery.”
The pilot will run for a minimum of six months, and marks an important step toward reimagining how community-based care can support the transition from homelessness to sustained independence.
